- Obtain the prescribed application form from R. No 15, Cash Section (Head Clerk-Cash) of LTMG Hospital & Medical College on working days between 11.00 am to 3.30 pm.
- Fill out the application form in your own handwriting with a recent passport size photograph affixed.
- Attach all required attested photocopies of documents including educational qualifications, birth certificate, caste/domicile, MMC/MCI registration, MS-CIT certificate, etc.
- Submit the completed application to the Central Dispatch Section of LTMG Hospital & Medical College, Sion, Mumbai 400 022 on or before 16.10.2026 up to 4.00 pm.
